Who We Are (PCPCC)

Patient Centered Primary Care Collaborative - 501(c)(6)
The Patient Centered Primary Care Collaborative (PCPCC) is a coalition of major employers, consumer groups, patient quality organizations, health plans, labor unions, hospitals, clinicians and many others who have joined together to develop and advance the patient centered medical home (PCMH). The Collaborative has well over 900 members.
The Collaborative is governed by the Board of Directors, the Executive Committee, and the Advisory Board. The Board of Directors has 10 members and meets bi-monthly. The Executive Committee currently has over 50 members and is involved in every aspect of the Collaborative. Members of the Executive Committee, along with guidance from the Advisory Board, direct the course of the Collaborative, run the Centers and the Taskforces, and plan the many conferences hosted by the PCPCC. The Collaborative believes that, if implemented, the patient centered medical home will improve the health of patients and the viability of the healthcare delivery system. In order to accomplish our goal, employers, consumers, patients, clinicians and payers have agreed that it is essential to support a better model of compensating clinicians.
Compensation under the PCMH model would incorporate enhanced access and communication, improve coordination of care, reward for higher value, expand administrative and quality innovations, and promote active patient and family involvement. The PCMH model will also engage patients and their families in positive, ongoing relationships with their clinicians. Further, the PCMH will improve the quality of care delivered and help control the unsustainable rising costs of healthcare for both individuals and plan-sponsors.

Patient Centered Primary Care Foundation - 501(c)(3)
The Patient Centered Primary Care Foundation (PCPCF) engages in education of the PCMH model through public conferences, webinars, policy papers, guides and toolkits. The Foundation will continue to conduct research and carry out activities to develop ways to assist all sectors of the healthcare delivery system in order to implement the PCMH model. Also, the PCPCF will continue to produce brochures that are consumer friendly which patients can use to ensure that they are receiving the best possible healthcare from their primary care providers. Finally, the PCPCF supports independent research on healthcare reform and the effectiveness of the PCMH model. The research is intended to develop methodologies to provide better outcomes for patients, more efficient payment to physicians, and better value, accountability and transparency to purchasers and consumers of healthcare, a charitable purpose.
